NEW APPLICATION - FOG 16.5 Discharge Permit Application Form
Applies to first time applicants. A first time applicant will need to also provide a receipt of grease trap cleaning service by a licensed grease trap cleaning professional with grease trap cleaning images, a cleaning manifest, and a detailed report of service. The initial application will be more involved and ask for detailed business information, a plumbing flow schematic, and other general business inquiries.

RENEWAL – FOG 16.5 Application Form
A renewal application applies to already permitted applicants. To complete the application an applicant will need to also provide a receipt of grease trap cleaning service by a licensed grease trap cleaning professional with grease trap cleaning images, a cleaning manifest, and a detailed report of service.
